I got hired at HD Supply company and as a new hire they gave me (4) Insurance policies; and one (1) policy that was $12.00 per pay period. For a total amount of coverage of $240,000. the brochure they gave me stated the premiums will never changed for life: that I could keep the insurance even if I left the company. The only way to loose the insurance was if I was fired. so I signed up for all (5) of the insurances: and I received conformation letter stating I was covered for $240,000. At first, I wanted the policies so I could make out my will, and I needed copies of the insurance to leave with instructions, and one policy for each of my grandchildren, and one for the church. but they never gave me a copy of the policies, so I got a lawyer. Attorney James lee and I signed a release of information but they never provided the policies and I showed Attorney the brochure; but he didn't want to take them to court and make them give me the insurance. I want my insurance they promised for my grandchildren!

I need to sue Columbus Area, Inc. 1515 East Broad Street: for malpractice, the tried to diagnose me without proper cause; they tried to have me probated, and tried to force me to take medication I don't need. When I asked Dr. Lee & Dr. Bosabes what they were typing on my chart they lied to me, and the nurses wouldn't let me see it, and I called police and police woman assaulted me verbally by making threats of harm to arrest me, and throw me off the property.

I need to Sue the State of New York; for sending my Son to prison on a parole violation for three months that have turned into too many years; pass his maximum sentence date, and he has been injured, permanent damage to his brain, tazed him (31) times and throw water on him.

Number (2) case; Bexley Police Arrested me without a warrant without probable cause, 1-01-2013 and caused me to loose my job. Bexley Prosecutor ordered me a mental evaluation and when I got there the whole building was empty; and I recorded the session, and they decided not to evaluate me as mentally insane: or try to keep me. (Shock treatment) at court they withdraw there charges of violation of protection order. I suffered my picture in a news paper of criminals for (2) weeks, and I got inquires from people asking me what happened. They labeled me, and my arrest record is polluted. Bexley will not give me a copy of the arrest, I asked police officers to bring it to court, and I requested the chief of Police by subpoena and they never gave it to me, the bail bondsman still needs to be paid, and I lost a years wages because I didn't get another job until October 2013. My son had the option of getting out of prison for $3,600; and I couldn't pay it, and now he's injured permanently from being tazed (31) times by guards and they throw water on him,
